Last month, I crossed the Walnut Street Bridge in Chattanooga on the way to check a delivery at a rehab clinic. The clinic had ordered a shockwave therapy device, a set of surgical stapler reloads, and several packs of surgical drapes. The front desk said, “Everything arrived.” I still opened the boxes. That is not pessimism. It is the job.
I work on the quality/compliance side of a medical equipment distributor just outside downtown Chattanooga. I review roughly 200 unique items a month, from disposables that cost less than a sandwich to powered devices that need a service contract. I don’t have hard data on how many incoming shipments have hidden issues, but I can tell you anecdotally that “complete” and “correct” are not the same thing.
This checklist is for practice managers, procurement leads, and clinicians who accept deliveries for a clinic or surgery center. It is not a substitute for your biomedical team if you have one. It is the basic walk-through I use before I sign off on an order.
Step 1: Confirm the clinical setup before you open the seal
The most common mistake I see is not a broken device. It is a product that works but does not fit the way your team actually practices.
For a shockwave therapy device, the first question is not “Does it hurt?” It is: radial or focused? What applicators are included? What settings will your rehab team use for the conditions they treat? If you are buying a system for plantar fasciitis and someone expects it to cover shoulder tendinopathy with the same head, that mismatch will show up after the return window closes.
For a surgical stapler, write down the intended procedure and tissue type before you call the vendor. Cartridge size, staple leg length, and handle configuration are not personal preference. When I receive a surgical stapler order, I compare the item code against the purchase order, but that only works if the purchase order was built from a clear clinical request.
What is a surgical drape? This is the one product category where people gloss over Step 1. A surgical drape is a sterile barrier used to isolate the surgical site and preserve the sterile field. It is not a patient gown, not a pack wrap, and not a furniture cover. If your inventory person says “it’s all blue cloth,” stop and get more specific.
Step 2: Read the label as if the label is the product
In November 2024, I received a carton labeled as the correct surgical stapler. It was the correct outer box. Inside the box, the reload cartridges were an older generation. They were not compatible with the handle the clinic already owned. We caught that because we read the label on each reload before opening the sterile package.
For surgical drapes, the label needs to tell you the fluid barrier level. In the U.S., many drapes reference AAMI PB70 levels, with Level 1 providing the lowest liquid barrier and Level 4 giving higher protection for high-fluid procedures. If the package does not state a level, ask why. A drape used for a knee replacement with heavy irrigation should not be a Level 1 sheet that was never designed for that exposure.
Also check: manufacturer name, catalog/model number, lot or serial number, expiration or sterilization date, and the words “single use” or “reusable.” If the vendor thinks a catalog number is all you need, that is a warning sign.
Step 3: Inspect the package on every carton, not just one sample
When I started in this role, we did not have a formal incoming inspection step. The third time I found water damage on the bottom of a carton, I made it a rule. Now every delivery gets the same treatment: check the pallet, check the bottom layer, check the corners, and make sure the tape has not been cut and resealed.
For sterile items, packaging is not packaging. It is the product. If a peel pouch or Tyvek seal has a pinhole, a crease, moisture, or an edge that lifts, the item is nonsterile. That applies equally to a surgical drape and to a sterile surgical stapler. I do not have statistics for how often that happens in transit, but I do have a memory of one rejected carton that turned into a $22,000 redo because the manufacturer had to reproduce the lot.
Step 4: Test powered devices with the correct setup
If you are buying a shockwave therapy device, don’t accept it based on a “demo feel” alone. The box operation should be simple: record serial number, check shipping damage, verify accessories, plug into a proper medical-grade outlet, power it on, navigate the menu, and run a test cycle at low intensity if the manufacturer’s IFU allows.
Check the serial number on the unit against the serial number on the certificate and the PO. If they don’t match, quarantine the unit. Also ask when it was last calibrated or safety checked. A new device should come with that documentation; a refurbished unit must definitely have it.
And please do not put a general-purpose power cord on a medical device. It is tempting to keep a drawer of random cords. I have seen a shockwave therapy device arrive with a power cord from a computer monitor. It fit. It was not the right cord. Step 5: Paperwork before shelf placement
I file the paperwork before the product moves into inventory. That is an unglamorous step, but it catches real problems.
For every shipment I need the purchase order, packing slip, item labels, the IFU or operator manual, and the manufacturer’s certificate or declaration if the product is regulated. If it is a medical electrical device, ask whether it is tested to IEC 60601-1. If it is a surgical stapler or another device that requires clearance in the U.S., ask for the FDA 510(k) number or cleared indication.
We didn’t always have a formal verification protocol. I assumed a matching PO number meant the order was right. That assumption failed once when we discovered that the packing slip covered the cartons we opened, but not the cartons still waiting in the hall. Now I match each carton label to the packing slip line by line before I sign anything.
Step 6: Set your rejection criteria before the truck arrives
A good receiving process is not a process at all if you make it up while the driver waits. State your red lines in advance:
- expired sterilization date or expiration date
- damaged sterile barrier
- missing serial number or mismatch
- incorrect product code
- missing IFU or documentation
- powered device that does not complete the startup self-check
If an item hits any of those, it goes to quarantine. It does not go to the shelf. The vendor can do a redelivery or a credit. We handle it after the shipment is documented.
Final note: this is local and boring on purpose. I would rather spend an afternoon on the Walnut Street Bridge in Chattanooga than in a supply room with packing lists. But a walk across that bridge is only enjoyable because the structure was inspected and maintained. Medical supplies deserve the same kind of ordinary care.
The boxes look fine most days. On the day they don’t, you want the habit already in place.
